The Problem: Physician Burnout
A large regional hospital (800 beds) was suffering from severe physician burnout. Internal surveys revealed that doctors were spending an average of 45 minutes on electronic health record (EHR) data entry for every 60 minutes spent facing patients. This "pajama time" (typing notes at home after shifts) was destroying morale and limiting patient intake capacity.
The Regulatory Context: GDPR 'Special Category' Data
The hospital's Innovation Lab identified Ambient Speech AI as the solution. By keeping a microphone on during the consultation, the AI could automatically draft the clinical notes.
However, Patient Health Information (PHI) is classified as Special Category Data under Article 9 of the GDPR. The Approach: Zero-Retention Local Transcripts
The hospital required a two-step AI pipeline:
- An open-source audio transcriber (Whisper) to convert speech to text.
- A generative LLM (Mixtral 8x22B, fine-tuned on Dutch medical terminology) to format the raw transcript into a structured SOAP (Subjective, Objective, Assessment, Plan) medical note.
This entire pipeline had to execute inside a "black hole"—an environment where the data was processed and instantly obliterated, with zero memory retention.
The NeuroCluster Solution

- Open-Weight Autonomy: The hospital utilized specialized, fine-tuned Llama 3 models specifically trained on Dutch medical terminology, ensuring far higher accuracy than generic public models without sacrificing data ownership.
